SAN ANGELO, TX — The Charleys Philly Steaks restaurant at Sunset Mall has been permanently closed by the San Angelo-Tom Green County Health Department after a failed inspection uncovered a roach infestation and slime buildup in the restaurant’s ice machine.
According to the official inspection report, the eatery scored 55 out of 100 during a routine visit on April 8, 2025. Inspectors documented numerous critical violations, including improper food temperatures, failure to sanitize contact surfaces, employees not washing hands properly, and contamination risks from food being stored in unsafe conditions.
The most alarming discovery was noted as an “active roach infestation” in the ice machine, along with “slime inside the machine.” Water used in food preparation also lacked proper backflow protection. Records received by San Angelo LIVE! state that the restaurant needs to be fumigated and that “everything needs to be removed.”
A follow-up inspection on April 28 confirmed the facility had not addressed the issues. The permit was officially pulled, and the report instructed that all equipment be removed and the facility deep cleaned. The inspector also noted that “management did not show up” for the reinspection and that the facility was “permanently closed by health.”
Health Services Director Sandra Villarreal confirmed the closure in a June 25 email, stating it was due to “unsanitary conditions of the establishment and a very low score.”
This marks the second closure for the Sunset Mall location. In 2022, the U.S. Department of Labor cited the business for underpaying workers and violating federal child labor laws.
As of June 25, the restaurant remains closed and is no longer licensed to operate in Tom Green County.
